Appointments

First Time Office Visit

Please arrive 15 minutes early for your appointment to allow enough time to complete the necessary insurance and patient registration forms. Please bring a list of all current medications. If this is a consult, please bring your referral (if required) and any appropriate films, reports and lab results.

Phone Advice

Our office handles routine phone calls from 8:00am - 4:45pm Monday-Thursday and 8:00am - 3:45pm Fridays. Please make non-emergency calls during our regular office hours so that we have access to your medical records.

A doctor is on-call every evening, including weekends for hospital emergencies. If you have a medical emergency after office hours you must go to the nearest emergency room. The answering service takes all calls after office hours. We cannot treat patients over the telephone. In case of a life-threatening situation, please call 911.

Medication Refills

We are happy to order appropriate medication refills. The quickest way to have your prescriptions renewed is for you to first contact your pharmacist or take the medication bottle to him/her, and then have the pharmacist call our office for renewal information.

If you contact our office to get a prescription refill, please have the medication name, RX number and pharmacy telephone number ready to give the clinical department. Please allow a 24-hour turnaround for prescription refills. We cannot refill medications after hours or on weekends.

Patient Reminders

  • HMO and Managed Care patients must call our office 3-5 days in advance for in-network referrals.
  • Please allow 7-10 days for completion of disability or other personal forms. There is an administration fee determined by applicable form.
  • For the convenience of our patients, a blood drawing station is located in our office and is available for patients from 8:00am - 4:00pm.
  • Parking on premises.
  • Co-pays and non-coverage fees are due and payable at the time of service. We have insurance billing.
  • We honor all major health plans.
